Before you enroll.
- Review our online provider directory to see if your doctors or specialists are in our network.
- Explore pharmacy services to see whether your current medications are on our prescription drug list and find nearby in-network pharmacies
Ways to enroll.
- Apply online (enrollment opens October 15, 2026)
- Mail your application. Download the Enrollment Application and Pre-enrollment Checklist. Return completed applications by mail to: St. Luke’s Advantage Rx, P.O. Box 1739, Boise, ID 83702.
- Talk to our sales agents.
Call: 833-840-1216 (TTY users call 711) - Visit with a local insurance broker. You’ll be asked to sign a Scope of Appointment form required by Medicare. Signing it does not mean you have to enroll.
After you enroll.
- You'll receive a confirmation letter that your application has been approved.
- We'll mail your member ID card to your home prior to the effective date of your coverage. This card covers your medical, pharmacy, vision, hearing and dental services.
- If you’re new to Medicare coverage, you can schedule a “Welcome to Medicare” preventive care visit within your first 12 months.
