Have a complaint?

St. Luke’s Health Plan is dedicated to your satisfaction and providing you with the highest quality of care. You may discuss any issues related to your care with your primary physician first or call customer service. This will often solve the problem. If you are not able to resolve your concern and/or if you were denied coverage for a service, you can request an appeal or file a grievance.

View appeal and grievance request forms or call customer service. Use of the form is not required, but it is a helpful guide for members to provide the necessary information, so St. Luke’s Health Plan can render the most accurate and timely appeal determination.

Appeals or complaints to the Centers for Medicare & Medicaid Services

  • If you disagree with a coverage or payment decision by your Medicare health or drug plan, you can file an appeal. Learn more about filing an appeal.
